Direction for the Application Form
 
1.    申请内容应完整、清楚、不得涂改。所有申报材料应用A4规格纸打印，并加盖单位公章;
  Contents of the Application Form should be complete and distinct, and  
   should not be altered. The information materials should be printed on  
   A4 paper and with the official stamp of the applicant;
 
2.    填写此表前，请认真阅读有关申报规定。未按申报要求申报的产品，将不予受理;
  Please read the relevant regulations carefully before filling the
   Application Form. The declaration which is not in accordance with the
   regulations should not be accepted;
 
3.      产品类别请填写是否具有特殊功效 (如：“防晒”、“育发”等）.   Please fill in the item “Product Class” here with“General” or “Special” class for   the product intended for special use. (eg. "sunscreen”, “grow hair” and so on )

Information materials required（all the foreign materials should be attached with Chinese Translation ）:
 
1.        产品配方;
     Formula of the product in concern;
2.   功效成分、相关证明材料及检验方法；
     Name of the active ingredient of the product, appropriate testimonial
     materials，inspection method;
3.   生产企业产品质量标准；
     Manufacturer's quality standard;
 4.   产品在生产国（地区）允许生产、销售的证明文件；
     Certificate of production and sales approved by the country of
     origin;
5.   化妆品标签样张或有效照片六套；
     6 sets of specimen or photograph of cosmetics labels;
 6.   申请出口化妆品标签审核时，必须提供进口国（地区）对化妆
     品标签的有关规定;
     Cosmetic regulations of the import country for the product intended
     to apply for export cosmetic labeling verification;
7.   使用说明书及使用警告；
 Instruction Manual and Warning;
8.   其它必要的相关材料；
     Other information (if necessary);
9． 需要提供完整包装样品共6份(5g或10ml以下样品,须满足检验
    用量)。
6 samples with package for test. ( The applicant should offer enough
samples, if the sample’s weight is <5g or capacity <10ml.)
