Guide to Application for Inspection and Quarantine of Entry Goods
 
I. Requirements for Inspection and Quarantine Report
 
   1. The applicant unit shall handle the registration with inspection and quarantine authorities before inspection and quarantine formalities.
  
   2. The staff handling inspection and quarantine formalities must be the inspection applicant qualified upon training by inspection and quarantine authorities and obtaining the certificate for inspection applicant.
  
   3. The inspection application agencies must handle registration with entry-exit inspection and quarantine authorities as per stipulations, and the inspection applicant must be qualified upon training and obtaining the certificate for inspection applicant agent before handling the inspection application formalities. An inspection application agency shall, when making the inspection and quarantine report, present the power of attorney on the inspection and quarantine.
 
   4. The inspection applicant must strictly observe the relevant provisions in applicable laws and administrative regulations, and may not forge, trade, change, alter or misappropriate the certificates and documents, stamp, mark and seal of the entry-exit inspection and quarantine authority, or otherwise shall be punished in accordance with laws and regulations upon discovery. 

   5. Generally, the entry commodities shall be applied for inspection and quarantine at least 20 days before validity period of claim. The imported bulk agricultural and sideline products (grain, oil and beans) and timber shall be applied for inspection and quarantine at least 7 days before entry.

   6. For import of microorganism, body tissues, biological products, blood and its products or breeding stock, poultry and its sperms and  embryo, the inspection and quarantine shall be applied at least 30 days before entry.
 
   7. For import of other animals, the inspection and quarantine shall be applied at least 15 days before entry.
 
   8.For import of plant seeds, seedlings and other breeding materials, the inspection and quarantine shall be applied at least 7 days before entry.
 
   9. An inspection and quarantine application form is limited to the same batch of goods under the same contract, invoice and bill of lading.

  10. The completion and selection/fill-in of inspection and quarantine materials must be orderly and legible, and may not be altered.
 
   II. Documents to be provided for inspection and quarantine application
 
   1. Trade and transport documents such as the contract, invoice, bill of lading and packing list.
 
   2. For goods subject to safety and quality license, sanitation registration and inspection and quarantine review and approval, the relevant certificate shall be provided.
 
   3. For quality inspection application, other documents shall be provided: foreign quality certificate or warranty, product user manual, and relevant standards and technical materials. If the contract is concluded based on samples, the transaction samples must be attached accordingly. If the price is settled on the basis of grade or measurement, weight survey shall be applied simultaneously.
  
  4. For application for inspection of entry waste, the Imported Waste Approval Certificate issued by the state environment protection department and pre-shipping inspection certificate issued by an inspection and quarantine authority recognized by entry-exit inspection and quarantine authority shall be provided. 

  5. Those applying for survey on damaged goods shall submit the relevant documents / certificates, such as tallying sheet of damaged goods, the railway commercial records, the air accident records or the marine reports;
 
  6. Those applying for weight (quantity) survey shall submit the description of weight and tallying sheet, etc.
 
  7. Import commodities subject to the inspection and acceptance by the receivers and end-users, or subject to the testing by other units, are required to be attached with the inspection and acceptance report, test results and weight breakdown.
  
  8. For entry animals and plants and their products, official inspection and quarantine certificate issued by the exporting countries or regions shall be provided; for those that need to handle the entry review and approval formalities, the license for entry animal and plant quarantine shall be provided. 
  
  9. In case of application for inspection of transit animals and plants and their products, the bill of lading and official inspection and quarantine certificate issued by exporting country or region shall be presented; In case of exit of animals, the Animal Transit Permit issued by state inspection and quarantine administration shall be presented.
 
  10. For application for inspection of entry transport tools, relevant inspection and quarantine certificate shall be provided.
  
  11. For companion animals carried by entry passengers and transport staff, the entry animal inspection and quarantine certificate and vaccination certificate shall be provided.
 
  12. For import of any article that is prohibited from entering the country due to scientific research or any other particular need, the approval certificate of concession shall be provided.
 
  13. For import of special articles, approval documents or specified documents as necessary shall be provided.
 
  III. Notes to Completion of Inspection Application Form
 
  1. Inspection Applicant Unit (affixed with seal): Complete the full name and affix with the seal of inspection applicant unit. 
 
  2. Registration No. of Inspection Applicant Unit: Complete the registration with the local inspection and quarantine authority where the inspection applicant unit locates.

  3. Contact: Complete the name of the inspection applicant of the inspection applicant unit.
 
  4. Telephone: Complete the working telephone number for contacting inspection and quarantine in inspection and quarantine applicant unit.
 
  5. Inspection and Quarantine Application Date: Complete the date of inspection and quarantine applied to the inspection and quarantine authority by the inspection applicant unit.
 
  6. Consignee (Chinese/foreign language): Complete the buyer’s name as listed in the foreign trade contract/foreign invoice.
 
  7. Consignor (Chinese/foreign language): Complete the seller’s name as listed in the foreign trade contract/foreign invoice.
 
  8. Name of goods: Complete the name of goods as listed in the foreign country (region) contract or invoice. (Standard name shall be maintained in the translation)
 
  9. HS code: Complete the commodity code as per the Catalogue of Commodities under Customs Statistics of the People’s Republic of China.
  
 10. Country (region) of Origin: Complete the country (region) name of place of origin for the goods as listed in the contract.
  
 11. Quantity/weight: Complete the quantity/weight as per the actual conditions of the goods applied for inspection. The weight includes gross/net weight.
 
 12. Total Goods Value: Completes the goods value (expressed in USD) as stated in the invoice. Prices in non-USD currency shall be completed upon conversion into USD.

 13. Package type and quantity: Complete as per package type and piece number for transport.
 
 14. Name and number for transport tools: Complete the name of transport tools for shipping goods, such as Seafreight No. XXX, Airfreight No. XXX, Train No.XXX, Seafreight Container XXXXXX.
  
  15. Contract No.: Complete the foreign trade contract No. for the lot of goods applied for inspection.
 
  16. Terms of Trade: Complete General Trade, three types of processing plus compensation trades and other terms of trade.
  
  17. Trade country (region): Complete the country (region) of the seller registration as stated in the contract. 
 
  18. Bill of lading/shipping document No.: Complete the bill of lading/shipping document for the lot of goods applied for inspection.
  
  19. Arrival Date: Complete the import date (or port arrival date) as stated in the advice of arrival for imported goods.
  
  20. Departure Country (Region): Complete the country (region) of the place for shipping as per the bill of lading.
  
  21. License/approval No.: Complete the quality license No. or entry approval certificate No. of the lot of imported commodity.
 
  22. Unloading date: Complete the date of unloading as stated in the advice of arrival for imported goods.
 
  23. Departure port: Complete the loading port as stated in the bill of lading.
 
  24. Entry Port: Complete the name of domestic unloading port.
 
  25. Claim validity period to: Complete the date as per date agreed upon and reckoned in the contract.
  
  26. Stopover port: Complete as per the bill of lading, and express as XXX for through route.
 
  27. Destination: Complete the place for use of goods or storage place before sales of goods.
 
  28. Container size, quantity and No.: Complete the container size, quantity and container No. for the lot of goods. If the containers are in great number, additional page can be used.
  
  29. Special provisions and other requirements for contract conclusion: Complete the special provisions and other requirements regarding inspection and quarantine when concluding foreign trade contract, and express as XXX if no such provision or requirement.
 
  30. Goods storage location: Complete the place of storage upon unloading of goods.
  
  31. Purpose: Complete truthfully as per actual conditions.
 
  32. Accompanying documents (mark √ or fill in): Tick the names of relevant documents and certificates issued by the inspection and quarantine authority as required by the inspection applicant unit according to those listed in the column, and write in the number of originals and copies, which are no more than one original and two copies. Two originals are permissible under special circumstances (state the reason, and present upon consent of the inspection and quarantine authority), and fill in the name of relevant documents and certificates that are required to be issued by the inspection and quarantine authority.  
 
  33. Marks and numbers: Complete the mark for transportation as listed in the invoices, packing lists and bills of lading. In case of any inconsistence between the documents, the invoice shall prevail.
 
  34. Solemn statement of inspection applicant: The inspection applicant solemnly signs in this column upon careful reading to indicate that he assumes the corresponding legal responsibility.
 
  35. Receiving certificate and document: The inspection applicant shall fill in the print no., date and name of the certificate and document as received by him in this column upon receiving relevant certificates and documents issued by the inspection and quarantine authority.
