Processing Authority: Department of Supervision of Shanghai Quality and Technical Supervising Bureau

Application Formality:

1. Written application letter

2. Application Letter for Appraising Approval, attached with quality handbook, basic information of the organization and reports of construction preparation, item list of the products applied for supervising authorization.

Processing Procedures:

1. Examine the written application;

2. Research and fieldwork according to the developing plan of quality supervising institutions, and notify the decision to the applicants;

3. If the application is approved, the applicant should prepare for the construction according to GB/T 15481 -2000 Universal Requirements on The Ability of Appraising and Calibrating Laboratory after receiving the notice from the authority;

4. The authority will organize experts to appraise the applicants when receiving Application Letter for Appraising And Cognizance;

5. Audit the appraising report submitted by the expert team and relevant documents and make decisions of approval or not. For the applicants who pass the audit, the authority will provide authorization formally. For non-qualified applicants, the applicants will be required to restructure in a limited time. If the applicant still cannot meet the requirements in half a year, his applying quality will be cancelled.

6. The principal attaches importance to the organization, and has enthusiasm to take the supervising and appraising tasks.
