Notes for Entry-Exit People＊s Health Examination

1. For health examination and inoculation, the applicants should submit their passports, ID cards or other available certificates and 2 pieces of 2 inches color photos without hats taken recently.
2. The applicants should have a good rest and not sleep too late, drink or do fierce exercises the day before health examination. Don＊t eat anything in the morning that they shall have health examination.
3.Please fill out the Check-Up Form conscientiously and accurately, including tell us your personal case history, your family case history and inoculation taboo.
4.The time for health examination: 8:00求12:00 a.m. every workday from Monday to Friday (excluding legal festivals and holidays). Special occasions shall be arranged by quite other considerations.
5.The site for health examination: NO.5 Hexiang Road, Zhengzhou City (The applicants should go to the cross of the east of Jinshui Road and the Zhongzhou Road, then turn to the east for about 300meters, and the hospital is on the north.) Buses of No.43 and No.26 shall take you there.
6.Personal health examination shall be operated at any time. And the applicants can obtain related certificates at the place of visa after 4:00p.m. of the 2
nd
workday after the Check-up. With respect to group health examination, the applicants shall make an appointment in advance and obtain the certificates 5 workdays after the Check-up.
7.The applicants, who shall go to the infected area of yellow fever, such as Africa and South America, must be vaccinated more than 10days (not including 10days) prior to their exit.
8.Contract workers, who shall go aboard, must go through health examination and visa formalities 40 days prior to their entry.
